If second or greater edition, enter as sequenced (i.e. 2nd, 3rd, 4th, etc.). If the book is a volume in a set, include volume number. Enter inclusive page numbers to indicate where the cited information appears.

If the publisher is inside the U.S. include the city a comma (,) and the postal abbreviation of the state. If the publisher is outside the U.S. include the city a comma (,) and the full spelling of the country.

Full name of the publishing company

If the publication is electronic and web-based, enter the URL for Web address. If a digital object identifier (DOI) is available, enter that in place of the URL.
